Request it - Subject access request
You have a right to ask us what personal information we hold about you and to request a copy of your information. This is known as a Subject Access Request (SAR). A data subject is an individual who is the subject of the personal data. If you wish to make a SAR, please complete this form and return to us by post or email.
1. Request
Make a subject access request by completing the form above or by calling, emailing or writing to SHP. We will need the information you need, and your:

telephone number

email address
We will also need to see proof of your address e.g. a utility bill. We cannot process your request until we have receipt of this.
2. Information supplied
We will make every effort to process your data subject access request as quickly as possible within 30 calendar days, if there is a reason we are unable to provide this within the timeframe we will be in contact at the earliest point.
